§ 20.530. Applications for leave.

  • All applications to the city council by any employee regularly employed under the jurisdiction of the city council for leave of absence with pay, in excess of vacation, incidental sick leave, death in family, and physical disability allowance, all as set forth in the rules of the civil service commission, shall be granted only when accompanied by the following documents:

    (a)

    A written communication from the head of the department with which said employee is connected, recommending that said application be granted;

    (b)

    Certificate of a licensed physician stating the nature and cause of the illness or injury;

    (c)

    Report of the city physician, or some physician designated by the civil service commission, giving the same information;

    (d)

    Affidavit of the employee stating that the employee was physically unable to perform duties and giving the name and address of the attending physician and the names of witnesses who can testify as to the illness or injury. (Code 1960, As Amend., § 547.010; Pet. No. 249854, § 58, 7-14-89)
