§ 263.30. Form of application.

  • The application shall be made in the names of both the corporation-employer and the employee in substantially the following form:

    Name of corporation       and       Name of employee

    (as employer and employee). The employee's knowledge, skill and experience, or, in lieu thereof, a certificate of competency, and the employee's job title shall be stated and described in the body of the application. The bond, where required, shall likewise be made in the names of both parties, as principals, and shall be executed by such employee and by two (2) officers of the corporation-employer on behalf of such corporation, and the bond shall have the corporation's seal affixed thereto and be duly acknowledged. Any such license, when granted, shall be so granted and issued in the joint names, and only one such joint license shall be issued for each employee possessing the qualifications as herein provided.
